Legacy Place

At Legacy Place we offer an alternative to traditional nursing home care. We specialize in care to individuals that would thrive in a smaller setting. Our desire is to develop a sense of community and mutual respect within our country home. We specialize in providing care to residents with different needs from mental health to physical issues.

 

Services provided:

• Bluestone Physicians Group offers monthly rounding.

• Individualized Care, based on care level assessed by RN 

• Registered Nurse On-Call 24 hours per day, seven days per week

• A staffing ratio that fits residents needs

• Medication Management provided by RN, in coordination with Pharmacy 

• Activities scheduled daily, with opportunities for outings outside the facility

• Menu planning for three home p meals with scheduled snacks daily

• Recreational opportunities 

• Spiritual programs available

• Laundry service

• Established relationships with multiple outside providers in order to offer PT/OT, Medical Equipment, Hospice, and many other services
